Betsy, I clearly remember one mother who had cracks around the base of each
nipple.  It looked as if her nipples were slowly being severed from the
areola.  Baby's latch was corrected at the first consultation, but the
nipples failed to heal.  The solution for this mother was breast shells - to
keep clothing off her very sore nipples and an antibacterial topical
ointment.  (APNO was suggested, but she preferred to try a single medication
preparation.)
