The Russian word for lactation consult is "konsul'tant po lactacii"
(consultant in lactation).  There is no set way to translate IBCLC.  One
can come up with several possible translations, but I would suggest
including a footnote with a more detailed explanation what it is.

Milkmaid is "doyarka", which has nothing to do with a consultant.  It is
definitely a woman who milks cows.
