No need for hooks (or for a lot of money).  But a cheap, fairly snug sports
bra, preferably one that zips up the front so you can put on/take off like a
jacket.  Costs around $10.

Cut a hole over each nipple, starting small and enlarging it just enough to
be able to slip the flange through.  The snug fabric holds the flange
smoothly in place, and the hole doesn't fray much over time even if left
raw.  One small-breasted mom felt she got even more milk this way because
the well-distributed pressure kept her whole breast in continuous, even
contact with the flange - something she'd had trouble doing on her own.
