Hi, Judy.
Our hospital uses a consent form (what you call permit) to cover formula bottles, pacifiers, nursery care, and nipple shields.  For convenience, we have these newborn topics on one form, but the mom only signs once, and only for the specific item that strays from hospital policy.  The only complaint we've ever heard is from one of our doctors who did not breastfeed her children.  When the information is presented in a kind, informative way, moms aren't offended.  At least not the ones I've seen.  :)  You might search archives for "consent" or "consent form" for more info.
