This does not sound like a plugged duct.  A plugged duct which is
*not* painful?  I agree with you that 48 hours is usually about the
longest it takes for a plugged duct to resolve, even if you don't do
anything.

I have seen things like what you describe before.  In one case we
never did figure out what it was, the mother having gone as far as
having a biopsy.  But I do not think this should be left.  Remember
that mammograms, needle biopsies and open biopsies do *not* require a
mother to stop breastfeeding.
