Jean,
Since I was working off the top of my head, I knew that my definition might
be off (the reason I did not post it to all of Lactnet!). So, I've gone back
to the source that I'm checking and will now officially describe my working
definition: Place both thumbs at the base of nipple, approx. 9:00 and 3:00.
Press in firmly towards chest wall, then gradually pull thumbs away from
each other. Repeat 'around the clock' to "stretch and release any adhesions
that may be pulling nipple inward."

Does that put us closer to the same page now?  Since I don't have the
article that you have, I would be interested in what Hoffman says
differently about the purpose of this exercise if not to stretch or release
(sounds like break to me) adhesions.
