I was plagued with tip plugs from the time my daughter was less than three
months old until she weaned (at 31 plus months).  I would often have several
per day - got in the habit of checking the tips of my nipples several times a
day just to catch them before the milk got really backed up.  My solution to
the symptoms was to manually express just at the base of the nipple, being
somewhat forceful about it (did this damage the tissue and make the problem
worse?).  I'd stand over a sink because once the plug was released, the milk
would shoot out for 30 seconds or so.  Often a second plug would come to the
surface and I'd express that out, too, followed by more milk.
