Hi Morgan

If the mother birthed at King Edward Memorial Hospital she can access the 
Breastfeeding Centre free and may be able to have baby assessed by 
ultrasound for tongue tie or other suck related issues. This is same 
location where Professor Peter Hartmann, Dr Donna Geddes and Dr Jackie Kent 
all work.

If "all concerned" suspect a tongue tie, why hasn't something been done 
about it?? There are good paediatric surgeons here in Perth and from what I 
am told tongue tie is being dealt with quite well here. This makes me 
suspect it's NOT the KEMH as well as other things you said and so someone 
needs perhaps to help her get in touch with another IBCLC.

There are also many very experienced IBCLCs here. I can provide you with a 
list if you wish. And there are several free through our health system if 
she is unable or unwilling to pay. Many of these will travel TO the mother 
in the early weeks.

I would think retained placenta has been ruled out??

Unless she knows someone personally I think she is unlikely to get donated 
EBM here - our milk bank here is only just started and all donated milk is 
being used with prem bubs at KEMH (to my knowledge).

Australian Breastfeeding Association is very active in Perth so if she 
needs/wants this support she can call the helpline 9340 1200 for 
breastfeeding counselling and to find her local group. Electric Breastpump 
Hire is also available through ABA.

From my own experience CST and Osteopathy seem fairly "new" here and not 
well known. She may not get a recommendation and may just have to select one 
locally and give it a go. Does she sound responsive to this suggestion??

Of course I agree the baby needs to be fed, at this stage it seems to be 
that this "milk" will have to be infant formula as there is no other 
alternative. If she is violently opposed to formula, is there a risk of her 
not feeding the baby?? She does need to realise at this stage there is no 
other alternative - baby needs to be fed. Are mum's nipples any less 
"hamburgerish" now than day 4 - might she use a supplemental nursing system, 
or if they are still too mangled could she finger feed while S2S Kangaroo 
Care?? This may be a really positive step for Mum??
