How many times in 24 hours does this baby nurse?  Waking at least once at 
night? One or two breasts at each feed?  Active, not cranky?
The 3 weeks back to birth weight is a red flag in my opinion.  Barely 3 oz 
per week is on the low side, but you have to look at the whole baby, not 
just pee and poop. A lot of pee and not much  gain could signal something 
like diabetes insipidus.

Then again.......
Just remember someone had to be at the very bottom of the weight gain scale 
to equal out the super gainers at the top of the scale.  Pat in SNJ, whose 
new great grandson is almost double birth weight at 2 mo :-(
