I had a dedicated mom who ended up with a failure to thrive infant at 5
months, ended up pumping and supplementing and struggling.  We couldn't find
a doctor to clip at all.
Her second child was also born with a tongue tie, started out with same
issues, sore nipples, not quite enough milk.  This time she just called and
called until she found a doctor to clip.  Her daughter was clipped at 2
weeks, no problems, cleared up all the problems and she is still nursing at
16 months, no further problems.
So you might want to check the first infant and see if this is the problem
from the beginning.  The mom I worked with will have someone who will clip
at birth for her next baby.
