Seeing the recent note about preemies prompts me to share my experience
here.
I have been most impressed by what I am seeing with preemies here in Peru.
Recently I was in the jungle region and saw babies that weighed just over 1
kilo being kept in kangaroo care by their adolescent mothers and nursing
every couple of hours.  The mothers would manually extract the foremilk,
then the babies would nurse to receive the hindmilk. Afterwards, they were
being fed the foremilk with droppers.  I did recommend that they set up a
type of nursing supplementer using a thin feeding tube with one end in a cup
of expressed milk and the other end attached to mom's breast so that babies
would receive more at the breast with less effort.  But these babies were
gaining.
I remember how dependent I was in the states on the Lactaid or SNS in these
cases. And I still think those are terrific products, but it is nice to be
able to substitute for mom's who don't have any possibility of purchasing
such a product.
