My wife Becky, RN IBCLC has just started working on a contract basis for our clinic and hospital to provide LC services.  Long story of how we got this far, but I will not labor that history in this post.

We are looking for someone on LACTNET that has access to a "patient satisfaction" type of survey form that we can give to the mothers after the fact so that they could provide feedback on the performance of the LC services.

I think it would be good to have this data gathered to present back to administration when our six month trial period is up.  We will then reevaluate the status and decide if they wish to continue some type of join program to provide LC services.

So if anyone has this type of survey and can email it to me or fax it or send it via snail mail.  The information to forward on is as follows:
