LLLI has a tear off sheet of this title.  You may find that it meets your
needs (and save you some time putting it together).  I believe it was put
together by LLLI to help deal with the "Insufficient Milk Syndrome" episode
a few years ago.  It is available through LLLI, or maybe a local Leader can
be of help.
