Group Health Cooperative (Pacific Northwest HMO) has a finger feeding protocol
in their breastfeeding management book.  Group Health recently entered into an
operating agreement with Kaiser, so that should make it "community standard of
practice" for you--if you stretch a little.  It is virtually the same as the
one in the Breastfeeding Answer Book.  E-mail me privately if you don't have
