Sorry, I accidentally sent my message before it was ready.  Here's the policy in
its entirety:
SPECIAL INFORMATION/PRECAUTIONS
        Indication is need to feed baby supplement without introducing rubber
nipple.
        The following procedure is for term infants.
EQUIPMENT/SUPPLIES NEEDED:
        Small cup with smooth top edge
        Cloth for catching drips
        Expressed breastmilk or breastmilk substitute
PROCEDURE:
        1.  Wrap infant securely with arms inside blanket and tuck cloth under
chin.
        2.  Pour 5 to 10 cc's of designated fluid into cup to start.
        3.  Hold infant in semi-upright position in arms or facing you on lap.
        4.  Bring cup to infant's lips and tip until a few drops enter infant's
mouth.  Let the cup rest gently on lower lip.  Once initiated, the infant will
voluntarily start to sip or "lap" the milk.  DO NOT pour or force the fluid into
infant's mouth.
        5.  Leave the cup resting on the lower lip until time to burp.
        6.  Lean infant's body back slightly after fluid enters his/her mouth and
make sure to allow time to swallow and rest.
        7.  Observe infant for willingness to continue feeding.
        8.  Burp frequently.
