I’m looking to see if anyone has studies or information regarding women who appear to have no colostrum at delivery. In the hospital, we get our patients started with hand expression within an hour of delivery if separated from baby, or later in the day if baby is sleepy or not latching. I’m wondering if others are having the same experience as us, but I’m find that a significant portion of women seem to be unable to express even a drop of colostrum for the first 1 to 2 days. I would guess maybe about 10% of women? I have no idea if it’s related to cesarean section, induction of labor, a prolonged labor or any other factor. I wonder if others are seeing the same. Would love to see some research on risk factors such as these and colostrum volume.
 
Do you also have a significant portion of women unable to get a drop of milk?
 
Have you seen any research about causes? 
 
What is your typical timeframe for beginning supplementation if there is no latch as well and unable to get any colostrum in a term, healthy baby and no latch?
 
Also, I’m wondering if anyone has seen any studies as to whether or not this correlates with low milk supply later?
 
For those with an antenatal milk expression program, do you ever see women who are completely unable to express colostrum at all during pregnancy?
 
If they continue with the expression do they start to get more milk during pregnancy? Perhaps this could help prevent this problem in the future.
 
I would say that I’ve had a significant number of patients who are unable to express any milk for 2 to 3 days and then mature milk production begins normally. Usually they have no risk factors for low supply.
 
  
 
Also, I am wondering if anyone else is noticing that many of the mothers have a decrease in milk production on the second day? Most, but not all seem to have a drop in milk volume before it increases around the third day. Is anyone else seeing this? I have a theory that this is why babies eat so much more frequently on the second day. I’m wondering if this is one of the mechanisms to increase mother’s milk supply and onset of mature milk production? Could the lower volume be triggering baby to feed more frequently and help onset of mature milk production?
