hello
although i'm a doctor, i would like to ask some questions who could point to 
non-medical causes:
1st of all, how many weeks is it now since she delivered?
2nd- you say she doens't have enough milk, do you base this on the fact the 
baby isn't growing or on the fact she expresses or pumps just 20 cc. the 
amount of expressed or pumped milk is not a good indicator of how much milk 
a woman has, as far as i know...
3rd- is she still in the hospital or she went back to her home? is she stressed 
or fine and relaxed? adrenaline can interfere with milk let-down.
4th- is she keeping baby a lot near her, is he carried? has she tried "marathon 
breastfeeding", forget pumps or whatever, just breastfeed on demand?
just a few, fast thoughts.

maybe this doens't help at all, but, well, i hope it does!
