Robert A. Hatcher is one of the co-authors of 'Contraceptive Technology', an
excellent sourcebook about contraception.  I'm not working in that area
these days so my most recent edition is now 11 years old.  There is an
entire chapter devoted to lactation with 53 references, and the information
on LAM, as well as info on the effects of other contraceptives on BF, are
clearly presented.  This was in 1991.
Hatcher & co are very committed to choice for women.  Their book has always
emphasized finding the contraceptive that is suitable for the individual
woman's needs, as she defines them, making it one of my personal favorites.

BTW, the only 100% effective method is total abstinence.  Not all tubal
ligations or vasectomies are successful either, and the 1991 edition of
Contraceptive Technology says that female sterilization has a 0.4% failure
rate (0.4 women per year will get pregnant for every 100 women who have had
tubal ligations) and male sterilization has a 0.15% failure rate (0.15 women
per year for every 100 whose partners have had vasectomies).
