Deborah:  It sounds to me like this mom is having MER problems more than
supply problems.  If this is the case, I strongly suggest the use of Bach's
Resque Remedy or NSP's glycerine based Distress Remedy.  Take it 30 min
before, 15 min before and then again immediately before latch on.  If she
can't anticipate that, then just right before.  If MER is the problem, then
this with Hops capsules can be very effective.  With MER, olfactory senses
are very important for effective triggering.  Have mom smell a piece of
baby's clothing that was "well worn."  I strongly push skin to skin contact
and massage for both MER and supply issues.

If supply is really the issue (then why is she having these plugged ducts?),
then the cabbage leaves can actually surpress lactation if left in place too
long.  For supply problems, I start with Fenugreek and move onto other herbs
from there (Blessed thistle, Goat's Rue, and others).  If mom perfers the
pharmacologic route, then Reglan is the drug readily available here in the
US, but I always caution about PPD (and rec against it in moms with hx of
any depression).  I refer moms to the pharmacy in Mexico for domperidone.
Most don't need to go this route if they use the skin to skin, massage,
frequent nursing, rest and herbs.
