I would think the difference is that when a baby is sleeping skin to skin on a mother's chest the baby is actually fairly upright even though the are chest to chest. 

To me this is different from being completely horizontal. 

Also, by definition a baby who is skin to skin is being closely monitored by the mother as opposed to a baby sleeping on a flat surface who is by definition out of physical contact with the mother. 

Just my thoughts.
