Here are some reasons there would be poor milk transfer:
premature or late preterm infant
tongue tie
other anatomical variations in infant (high or bubble palate, short tongue)
maternal anatomical variations: inverted or retracted nipples, too large
nipples,
"loose" and stretchy nipple/areolar complex
birth injury, facial/jaw assymetry, torticollis
low tone infant for various reasons
low milk production
