The Xerces organization lists the following as "Possibly Extinct: Missing; known from only historical occurrences."

Bombus franklini
Hylaeus anomalus
Hylaeus finitimus
Hylaeus gliddenae
Hylaeus mauiensis
Hylaeus nalo
Hylaeus niloticus
Hylaeus perspicuus
Macropis steironema opaca

Of course, they are named for the extinct Xerces Blue butterfly (Glaucopsyche xerces) -- "The first butterfly known to go extinct in North America as a result of human activities."
