Dear Jane;
        Maybe the Bees swarmed because the "hive" was "nasty"?          I mean
even us humans would swarm if our "hive" was "nasty".
 
        I would strongly recommend that you understand why there was Queen cell
on the front step of the hive before you requeen and have the same problem
again.
