    I just purchased a Beekeeper's Backpocket Reminder, by Michael
Rainville.  It contains great reminders in the front section (i.e. seasonal
checklists, equipment checklist, queen marking colors, installing package
bees, hive inspection and feeding guidelines, medications, diseases,and hive
management.)  The last half of the book contain blank templates for yard and
hive records.  I got mine on Ebay, where the author is selling them.  If you
go to Ebay and do a 'beekeeping' search, you will find his book.  Cost is
$5.95 plus shipping.  Best of all, it fits in the pocket of your beesuit.
This allows the beekeeper to take notes when he needs to, during hive
inspection.  I have also heard of beekeepers who carry small digital
recorders, transcribing data at home on the computer into Excel.
