"Feeding pattern normal freq"  This tells us that the baby is put to the breast frequently, but it says nothing about if the baby swallows, what the supply is like, if the baby actively feeds, etc.  Which all points to why the weight gain is very low.  

If the pump was semi-accurate @ 5mls per pumping, maybe the baby gets that at the breast, say, 10 times/day, plus 2 oz formula.  This adds up to less than 4 oz a day total intake.  Going to the hospital as others suggest isn't unwarranted.  I might try feeding the baby a physiologically normal amount away from the breast, in a way that doesn't burn too many calories as baby can't afford to lose them.  If baby was able to eat, I'd continue offering an appropriate, measured amount for one or two days and see if baby is capable of eating and gaining.  If baby was unable to eat or unable to gain, then I'd concur with the hospital route.
