"Stools are loose, yellow, sometimes explosive."

That's  normal though, isn't it?  Some times babies do 'explode'....and
babies sometimes do look as if they're straining...

Bleeding in the perianal area is not normal, of course.

Horrible thought, but have you ruled out abuse?
