I believe the widely spaced "perky" breasts ARE THE reason for the trouble 
here.  perky can mean that there is insufficient glandular tissue so the breasts 
point up rather than downward.  possibly very little glandular structure on the 
top of the breast.  I've seen this--it may be the main issue...
