I currently have a patient who has had idiopathic Diabetes Insipidus for the last 3 years.  She takes DDAVP (usually 1 pill but took 1/2 pill during pregnancy.  She is one week out after delivering 34 week twins and is making 13 ml every 3 hours.  Hystory of hypothyroidism as well. Looking at previous posts on this, discussion is that patients with DI need to never be without water...BUT when taking DDAVP, patients need to be cautious about water intake as they could become water intoxicated due to the severe reduction in urination and increased resorption of fluids. This mom knows how to manage her fluid intake to stay well hydrated, and keep normal lab values but I've had to adjust my teaching on fluid intake.  

Does anyone have additonal information regarding lactation outcomes for women with DI?  So far, this mom has an insufficient supply.  I am leary about recommending Reglan. 

Any information would be welcomed!
