I attended a conference this spring where Dr. Jain was a speaker.  She showed
her video on clipping frenulums, which I recommend.  The problem is I cannot
find her address.  She is the director of the Lakeview Breastfeeding Clinic,
Calgary, Alberta Canada.  Maybe some of the other members of Lactnet could
help you get more information on contacting her.
Good luck, we need more health providers who see the benefit and want to
learn this simple procedure.
