The suggestion that mentors should be paid for their time is an intriguing
one, coming from a medical model where interns and residents (and for that
matter, junior nurses) are *paid* (though not much!) during their training
time.  Mentorship is considered very prestigious, something to be cited on
a resume.

Mentors can utilize residents/research fellows to do the grunt work (note
writing, writing orders, making phone calls, etc.) and the mentees see
patients on their own and present them to the attendings, maximizing the
time spent by the attendings on each case.  The extra time spent on
education is more than equalled by the time saved by having another
person, particularly when that person is educated in how to triage
(handling easy, straightforward cases themselves with your approval, and
referring more complicated cases up the chain.)  They also benefit by
having their name on publications written by their residents/fellows as
the senior author.

In my readings of the requirements to sit for the exam, internship that
merely involves "shadowing" doesn't even count towards hours, so I would
think this type of active participation with independence would be
necessary.
