Hi Anne,
You are so right when you say "What about the near term baby
(35-37wks) healthy and on the ward with mum. He doesnt always
breastfeed so well."

I've also seen these babies (37 wks) called "early term" infants. I
believe everyone of these mom/baby couplets require an individualized
plan. And I believe all of them need an early f/u visit.
Some of the babies bf right away, vigorously, and continue to do so.
They may have moms who have a generous amt of colostrum. Some of the
babies do a percentage of "good" feeds and then sleep through other
feeds. Some of the moms have very little colostrum.
I don't automatically have mom expressing (by hand usually), although
I do talk to them about the possibility, depending on how often and
effectively baby bf's.
Remember, we as bf helpers are much more comfortable evaluating
feeding at the breast, and maternal behavior (does she do STS and
respond to all cues?) whereas other staff or doctors may be more
comfortable with measured amounts.
You can certainly effect change by tactfully sharing your knowledge,
experience, and what you are observing. Very often a baby will be let
alone by them to bf ad lib or to not be topped up after a particularly
good feed (observed/shared by you). Change will be gradual.
