Just my two cents...
I think that Linda Duce hit the nail on the head in her posting in which she
wrote: "...rather than labeling something a success or failure, how about
"unable to achieve latch related to ..." (fill in the blanks as needed
with...ankyloglossia, etc.) or breastfeeding terminated (sure there might be a
better word there) ? deferred? related to painful nipples, lack of support,
medical indication (list it), or whatever is appropriate?". When healthcare
professionals accurately document it is far easier to identify not only the
root of a problem but also the potential solutions. If a woman's breastfeeding
experience has simply been recorded as "breastfeeding failure" it is not
possible to identify the "why" of the situation or how to proceed. Did she
"fail" because the baby was tongue-tied or was it the result of quitting due
to severe nipple pain/trauma possibly resulting from a poor latch? Did the
baby fail to gain and was it ever determined why (inadequate milk transfer,
lack of supply due to reduction mammoplasty, etc.)? Are interventions designed
to boost supply, promote relactation, etc. appropriate?

documentation in charts!)
