Lisa is talking about a mom's lactiferous sinuses being farther back in
some women with large, soft breasts.  If a mom's L. sinuses are too far
back for a pump, how can a newborn baby get her mouth far enough on to milk
the sinuses?

I had a client who was having a lot of trouble with supply, both with baby
feeding and with pumping.  Mom said that it felt as if her L. sinuses "slid
around," sometimes being where baby could milk them and sometimes not. She
found it somewhat helpful to hold the breast with one hand, compressing
from the chest side of the breast, thinking to keep the sinuses foremost.

Can some babies (and pumps) just not get far enough back?  A good case for
hand expression, though I have a heck of a time getting moms to practice
enough to get good at it.
