OMG!  I was just informed (and so surprised ;) to hear that JCAHO core measures include % skin to skin, bfdg w/I 1st hr of svd and 2 hrs CS!  Our corporation requires report-out and we will be pitted against each other to improve!  In a query addressed to the L&D manager I was told, "Our nurses have become so comfortable with s-s that all mothers are told at delivery "open up your shirt, here comes the baby!"  I am shocked and pleased.  I never thought I'd see my screaming and jumping up and down pay off!!!!  I can retire happy.

Subject: 1st breastfeeding and supplementation

I have chuckled reading the posts today about 1st feeds and unnecessary
supplementation.  Kudos to you ladies!  I love what everyone has had to
say about leaving babies with their mothers skin2skin and all the evils
of hospital practices.  I am a hospital based LC and though we are doing
better now a days, we can still do better.  One thing on the horizon
(and I think it's a positive thing in the long run) is that JCAHO (the
Joint Commission on Accreditation of Health Organizations), or something
to that fact) the accrediting body for most hospitals has new 'core
measures' which just came out in January I believe.  These core measures
translate to a sort of 'report card' for hospitals. There are several
that apply to OB units and nurseries the most notable is EXCLUSIVE
breastfeeding rates.  At some point, we will have to actually track and
report exclusive breastfeeding. That means that if a healthy term
newborn receives anything but the breast or EBM, it will have to be
documented as to the reason why. So, in addition to I think Marsha's
idea of 'locking up the formula' in the Pyxis med station, this will
also make the staff think again about giving a supplement when they must
document WHY they are doing this.  Educating the staff and pediatricians
and the parents also goes a long way in helping with this too! Thanks
