We are working on outpatient charges.  Start date keeps getting
pushed further ahead as we run across little details we hadn't
considered.
However, inpatient charges are a different matter.  Are other
"consultative" services such as nutrition, social workers, etc
charged (as opposed to "therapy" type services)?  One could argue
which of those best fits LC services.
I think the biggest challenge, at least in our area, is that most
insurance reimbursed the hospital either:
a) - on a fixed rate per case (in other words $X for any vaginal
delivery and $Y for any C/S dellivery, regardless of what it
actually costs the hospital) or
b) - by a system whereby the insurance company says to the
hospital,"This is the group of individuals we cover who would come
to your hospital.. We will pay you such-and-such amount per year to
meet the hospital needs of the entire group regardless of what might
bring them to the hospital and how complicated the cases are."
Insurances that reimburse according to actual cost of each case seem
to be few and far between.  In the end it is the self-pay patients
that wind up bearing the brunt of costs since most of the
reimbursements mentioned above don't seem to come up to the actual
costs of care!
