As a HCP myself, I would much prefer to receive a report from the LC
whenever possible, both so I am aware of any issues (even if they have
simple fixes) and for education for myself.
I think Diane Wiessinger's idea of having "standard reports" to send out is
not a bad idea, also. Around here quite a few providers are terribly
uneducated about breastfeeding, but not necessarily unwilling to learn. For
them to read about even basic information like how to help a mom with
uncomfortable latch, or engorgement, or other standard simple issue would be
very helpful. We have a lot of providers who respond to even simple
problems with weaning advice. Also, for them to see a nice comprehensive
plan for a baby with issues, especially weight gain issues, might go a long
way towards encouraging them to refer prior to reflexively telling mom to
wean. Sending a report in a concise format raises the IBCLC's professional
appearance and might help the HCP view them more as a fellow professional.
