I don't see any evidence of thrush in her history, although it is tricky that the diflucan she is on now seems to be providing relief. My understanding is that many of these meds have anti-inflammatory properties and that might be what is helping her.
My thoughts would be to do a clean catch culture of her milk and possibly also of her nipple/areola skin. She has had this pain bilateral all along, it seems, and I am thinking strep or MRSA, or could be some other less common germ. She can go from there as to appropriate treatment which would probably be many weeks. Some of our bf-savvy doctors have posted of this treatment. In the meantime can she take the highest, safest dose of an anti-inflammatory med to dull the pain. Even a narcotic painkiller for a short time might help.
