Kathy Boggs and Dori Dorfman gave cogent explanations for why one is
supposed to follow the instructions exactly when mixing breastmilk
substitutes.  The other opportunities for error are from packing the powder
into the measuring spoon too firmly, or from not filling the spoon enough.

This is one of the things that has always scared me about using these
products.  When people whose literacy is not the best and whose
understanding of osmolality is minimal, are going to be mixing this up and
administering it to infants it is bound to go wrong every now and then, even
when the water is clean.  Parents who are going to be feeding babies this
way MUST have adequate instruction in how to do it, and not be left on their
own to figure it out as best they can.
