It could be muscle spasms in the pectoral muscle and other muscles in 
the back.  It can be from poor positioning (hunching while nursing) or 
sometimes from nursing lying down if mum if contorting herself in a 
funny position.

The Newman Breastfeeding Clinic and Institute www.ncbi.ca in Toronto 
(Dr. Jack Newman and Edith Kernerman) have done research on Mammary 
Constriction Syndrome - which involves muscle spasms in the pectoral 
muscles.  They have a series of stretches the mother can do to stretch 
out that muscle.
