My main concern is that I am frequently asked why doctors don't know this.

I say sort of the same (not much info in med school, too much info to  keep
  up with, too many new meds etc.) also why PDR says everything is
contraindic :-(  I give out Tom's web site and tell them how their dr can
get  own book so info is at his fingertips!  I also say same to pharmacists
:-)  Tom's book is inexpensive enough that there is really no excuse for
professionals not to have  a fairly new or newest ed.   Sincerely, Pat in
