Aloha,

I have mom's permission to post----I am stumped and hope that the collective
can help.
I just started working with this mom.   She just had baby #5.   A brief
history.
Baby #1 was born via unplanned C-section after determining he was too big.
This mom is just about 5 feet and very petite.   She had an overabundant
supply of milk--nursed for 1 month and weaned upon returning to work.

Baby #2 born via planned C-Section.   Her milk supply was good.   Not too
much as she says.

Baby#3 born via planned C-Section.   Mom says her supply was just right.

Baby #4 born via planned C-Section 7 years later.   Mom says she had terrible
engorgement.   She says she swelled up liked grapefruits and just as hard.
She worked with LC,   LLL Leader and Massage therapist--to no avail.   She
went to the hospital --they said this was the worse case of engorgement they have
ever seen.   Mom says they had her pump for 1 1/2 hours--all she got out was
1 ounce.   She gave up breastfeeding.

Baby # 5 born via planned C-Section on Sunday morning.   Baby #4 is now about
22 months.   Mom has not been able to nurse baby #5.   He was transported to
another hospital NICU.   Mom is sketchy on circumstances, but it had to due
with baby's lungs and baby should be home within two weeks.   Mom tried to pump
12 hours after birth with hospital grade pump.   She only pumped about 5
minutes per side and no colustrum.   She is home now and using hospital grade pump.
  She is pumping every 3 hours for 15 minutes down from 20 minutes.   Early
this morning she was like grapefruits again.   She has been using cabbage
leaves and cold compresses switching to warm compresses prior to pumping.   She
told me she has the vacuum suction on the pump up to maximum--if she doesn't, she
does not get any milk.   The most she pumps per session has been 3 ounces
combined.   She describes it as the milk not coming out.   It is there but won't
come out.   It does not spray out--only drops.   She has only felt one
letdown.

She has never had any breast surgeries or injuries--no medications that would
affect supply.
