Michelle, 
This baby is seriously underweight and has very little energy to transfer milk from the breast.  Mom needs to start pumping/hand expressing frequently, supplementing more, and helping that baby to grow so he can do a better job at breast. Baby should be nicely past birthweight by day 19. 

Pedi is negligent in only advising frequent weight checks.  Unless some intervention is made, baby will continue to falter and compromise breastfeeding altogether.
