I got a call from a mom with an 8 wk old.  She has been struggling with milk
supply issues the entire time.  She called me asking about domperidone.  She
has been working with an LC.  I asked the mom (as I usually do) for a
history - nothing out of the ordinary.  Normal, un-medicated vag birth -
nursing started off fine - no issues - just no milk.  No retained placenta -
no history of hormonal issues.  I asked if anyone had done a history such as
this - mom reported that NO - no one had taken the time to look for an
answer - just told her to keep pumping, try an SNS (miserable failure there
as she was given the unopened package to take home and try on her own.) and
take some fenugreek.  No one asked you any questions about your physical
history - hormone issues, breast injuries or surgery?  Nope.

 

So - do you have a history of breast surgery?  Why yes - says mom.  When and
why?  15 yrs ago I had implants - I was vain at the time and didn't like the
shape of my - as my Dr. called them - snoopy dog breasts.  Snoopy??  As in
long and tubular?  Why yes - they had a very wide spacing between them and
the areolas were bulbous and funny looking.  Dr said there just wasn't much
in them.  Hmmm.  Can anyone tell me why I am discovering a probable reason
for low milk supply over the darn phone?  Can anyone tell me why no one
asked these simple questions?

 

I discussed the possible reason of low milk supply due to possible
hypoplasia with this mom who was relieved to know that she actually had been
doing everything right (you wouldn't believe the efforts she has made!) to
have a milk supply.  She will be trying the domperidone to see if she can
get more than 4 ounces per day.

 

8 wks of frustration because no one asked about her history.I don't get it.
