My understanding of the fats in breastmilk is that the quantity is quite stable, 
the content quite variable depending on the mom's fatty acid intake.   Of 
course there are the fluctuations within a feeding, beginning to end.

My educated opinion of this lactose issue is that the diet would have to be 
pretty extreme to alter the fat/carb ratio very much.    Starvation or very 
prolonged unbalanced dieting maybe.   Just my 2 cents.
