Kathy, please discourage this pediatrician from doing a fat analysis of the
mother's milk and instead encourage this mother to work with you
The fat content of her milk is probably fine but with your description of the
baby spitting up, I wonder if this is more of an issue of the baby getting
more foremilk and not staying on long enough to get the hindmilk?  Having a
visit with this mother and getting a detailed history would help identify what
really is the issue here.
How well do you know this pedi?  Could you call them and describe the issues
of different fat contents of breast milk varying on time of day and time
between nursings?
Offer your help with this mother and maybe educate them at the same time.
