And then there is the fact that some tt's are snipped but not enough is released, or it was snipped and reattached and perhaps a lurking posterior tie is the culprit - and baby just can't use his/her tongue to draw nipple far back enough and keep it there.. that and/or a tight lip tie can cause continuing pain regardless of mother's best efforts at improving latch.. and regardless of lip tie and tongue tie.. persistently damaged nipples are at a high risk of infection. Hope this mom gets baby's oral anatomy assessed again. At the very least, the exercises in Cathy Watson-Genna's book, Supporting Sucking Skills, are a Godsend for helping babies learn to open wide and use their tongues and there is an exercise for babies whose parents choose not to have tt revision done ( or done again).  

Many moms I have worked with have really great success with baby having nipple and areola "poured" in and then leaning back quickly so they can't slip off.
