From an article from this month's eOB-GYN newsletter:

<A study published this fall strongly suggests that obesity itself can cause
maternal and perinatal complications, including stillbirth, preeclampsia,
and cesarean delivery (Lancet 2006;368:1164–70). . . . . . . . .

"It was [especially] unexpected that these relatively modest average
increases in weight" had such an effect on outcomes, Dr. Villamor said in an
interview.

While describing the study as "groundbreaking," Dr. Caughey noted that it
raises several questions that it was not able to answer.

For one thing, it's unclear whether the associations are related to actual
weight gain immediately before the second pregnancy or to a lack of weight
loss after the first pregnancy. There's also the possibility that
breast-feeding after the first pregnancy may play a role, since women who
breast-feed for longer periods of time have less weight retention and are
less likely to develop type 2 diabetes.>
