As they do with manipulated mom's milk.
The only way that fortifying or manupulating would not be needed is to have
mom create a double sized or triple sized milk yield and save the overload
for the time her milk changes and the child still needs premie milk. Then mom
will have to pump and store to keep her production going untill baby will be
fine on term milk at breast.
I think I would prefer to lactoengineer het milk (though that needs an
overproduction as well, but because baby will grow faster he will probably
also overgrow the need for special milk faster).
I, indeed, too, do recognise the fact that we do not know yet what exactly
lactoengineering does with the milk or the infant, but I tend to believe that
it will be preferable to fortifier.
