This sounds like a baby who may have GERD going on -- "reflux gone bad" as
Dr. Brian Vardebedian would call it, since we *all* have physiologic reflux,
and it only becomes problematic when it morphs into pathologic reflux.

Sounds like a lot of food allergy/elimination has been going on, but the
problem may not be with the content of the foods, but rather the baby's
ability to digest it.  I would take a long, detailed history of the baby's
feeding behaviors, and aversive reactions.  This sort of "getting worse over
time" makes me think GERD is worth investigating and ruling out.
