Hi Julie,
I can only speak as a nursing mothers counselor, since I just got IBCLC this 
year, but as a listed "free resource" who in the past spent hours on the phone 
with mommies, perhaps this will help. When I get a mom like this one, who 
really just needs a b/f buddy and maybe some new friends for whatever 
reason, I get her involved in a local b/f group, whether it is nursing mothers or 
LLL or whatever you have active in your area. You can also refer her to 
websites like Kellymom or LLL for information and encourage her to "get back 
to you" with what she found that worked for her. Chances are, it will have 
worked, her crisis will have passed, and she'll forget to call you.  Also, can you 
explain that you cannot continue to give out this free advice, since you could 
get in trouble because she isn't an "official client" who has seen you in-office 
or in-person. This affects your liability.  You can explain that if you were a 
midwife or pediatrician your professional credentials would not allow you to 
give out advice to someone like a fellow soccer parent who wants to ask you 
questions at the field during practice. For her best interest, if she wishes to 
continue, you have to make the relationship official, and this includes in-
person evaluation and permission to report any applicable findings to her doc 
or baby's ped. This way it doesn't seem like you are just all about the money.
